Summary:
Facilitates and supports timely and effective discharge planning/placement needs. Adherence to post discharge plan(s) of care and the holistic health and functioning of the patient/family. Serves as a member of the interdisciplinary health team. Collaborates with departmental/organizational support. Services. Assists in the development, maintenance and evaluation of the Palmetto Health Medical Management Social Work Services and performance improvements.

Responsibilities:
Provide psychosocial assessment and interventions. Coordination and collaboration with ancillary services, providers, and agencies.
Implementation/coordination of identified discharge planning/placement needs. Work closely with others in an effort to optimize quality and minimize cost.
Provide post-discharge follow-up when deemed appropriate
Assist in development/maintenance and evaluation of clinical data; interpret data and make recommendations based on results.
Utilization of paper and computerized charting.
Support product line/medical staff quality improvement initiatives.
Rotational weekend and weeknight calls.
Requirements:
Education: Master's degree from an accredited School of Social Work
Experience: 2 years of experience in a health related area
License, Registry, or Certification Required: LMSW, LISW; CPR or Heart Saver required.
Special Training: Psychosocial intervention skills; analytical and problem solving abilities; strong written, verbal and telephone communication skills; ability to work within interdisciplinary setting.
