The Records Analyst II provides follow up and enters data to meet the health care needs of children in foster care and assists with health care needs for families and children involved in family preservation cases.
Collaborates closely with the Regional Nurse Coordinator in gathering health care documentation and scheduling appointments.
Gathers and organizes medical documentation including after visit summaries, past medical records and immunization records and other health records.
Accurately and consistently enters documentation in CAPSS such as upcoming appointments, encounters, and follow-up appointments.
Collaborates with Regional Nurses and or Statewide Nurses to enter and follow-up on referrals and follow-up care.
Collaborates with case managers and supervisors to determine if a well child, dental or follow-up appointment occurred.
Requests after visit summaries.
Demonstrates a commitment to customer service, assists foster parents and case managers in scheduling appointments.
Prepares accurate and timely reports of due and overdue well child visits, dental visits, follow-up care needs, and other health care related reporting issues.
Monitors and follows up on communication from providers such as faxes and emails.
Acts as a resource to assist case managers, investigation staff and others on health care related data needs.
Will be required to perform Red Cross shelter/emergency response duties as directed.
Minimum and Additional Requirements Agency Requirements:
A High School Diploma and three (3) years of experience in records management, information systems or office administration.
An Associate's Degree and one (1) year of experience in records management, information systems or office administration.
