Clinical Pharmacy Specialist (Critical Care)

The Critical Care CPS functions at the highest level of clinical practice, working as an advance practice provider under a scope of practice as defined by the medical center to directly care for patients served in all critical care wards and any acute care wards or other areas as assigned. He/she will effectively and efficiently function as a member of the Intensive Care Unit (ICU) team. The CPS will serve as the ICU expert in all areas related to critical care medication therapy management and functions as a consultant to other health care professionals. The Critical Care CPS is an advance practice provider functioning to promote and manage drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The CPS must be capable of using professional judgment and clinical expertise to solve complex problems and to provide clinical information to both providers and Veterans. The CPS will participate in multidisciplinary rounds; perform initial and daily assessments of patients' medication therapy and make recommendations or changes (within scope of clinical privileges) as necessary; works closely with physicians and nurses to monitor patient response to medications-including efficacy, safety, and outcome; documents interventions and recommendations via progress notes and consults, reviews and develops ICU protocols to ensure the facility practice is current with all Critical Care guidelines and that protocols are being used appropriately by providers; serves as a clinical pharmacy preceptor for pharmacy residents and students; and other duties as assigned. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE:
Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.). Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of VA Handbook 5005. Physical Requirements. Must pass pre-employment physical exam. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Experience:
PGY-2 Critical Care or Internal Medicine or 3 years of autonomously providing comprehensive medication management in Critical Care/Internal Medicine under a scope of practice. Grade Determinations:
GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade and fully meet the KSAs at the next lower grade (GS-12). 1.Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
